包一，64排螺旋CT，1套；包二，磁共振，1套。
2、招标文件售价：333美元/每份或2000元人民币/每份。邮购标书另付100元人民币或50美元，售后不退。投标人购买招标文件时，需将标书款电汇至五矿国际招标有限责任公司四川分公司帐户，并提供以下资料：1、投标人营业执照正本、副，本复印件各壹份（加盖投标人公章）。2、投标人对购买人的授权书壹份（加盖投标人公章）。3、A. 如果投标人按照合同提供的货物不是投标人自己制造的，投标人应得到货物制造商或货物制造商在中国地区合法授权机构同意其在本次投标中提供该货物的正式授权书原件；或B.如果投标人按照合同提供的货物不是投标人自己制造的，投标人得到的是中国区总代理同意其在本次投标中提供该货物的正式授权书原件，同时还须提供货物制造商对中国区总代理的授权书复印件（加盖投标人公章）。或C. 如果投标人按照合同提供的货物不是投标人自己制造的，投标人得到的是项目所在区域代理同意其在本次投标中提供该货物的正式授权书原件，同时还须提供货物制造商对中国区总代理或项目所在区域代理的授权书复印件，和/或中国区总代理对项目所在区域代理的授权书复印件（加盖投标人公章）。4、购买人身份证复印件壹份（加盖投标人公章）。

Minmentals International Tendering Co.,Ltd entrusted by the purchaser, invites sealed bids from eligible suppliers home and abroad for the supply of the following goods and/or service by way of International Competitive Bidding and traditional bidding. The tender notice was released on www.chinabidding.com on Mar.30,2012
1.Name of Products/Equipment, Quantity, and Main Technical Data:
package 1，64-slice spiral CT，1set；package 2，Magnetic resonance，1set。
2.Price of Bidding Documents: Non-refundable payment of USD333 or RMB2000 for each package，an extra postage fee of RMB100 or USD50. When the bidder purchases the bidding documents, the bidder must pay the bidding documents fee to Minimetals International Co., Ltd. Sichuan Branch account by T/T and provide the datum as following: 1, one piece of the copy of the bidders oringel and duplicate business license original with the bidder’s common seal. 2, one piece of the bidder to purchases person's power of attorney with the bidder’s common seal. 3, A. If the bidder provide the goods is not produce by oneself according to the contract, the bidder should obtain original power of attorney from the manufacturer or the manufacturer in the Chinese area legitimate authorization organization agreed it. Or B. If the bidder provides the goods is not produce by oneself according to the contract, the bidders should obtain original power of attorney from the general agent in China agrees it and provide copy power of attorney of manufacturer to Chinese area general agent with the bidder’s common seal in the same time. Or C. If the bidder provides the goods is not produce by oneself according to the contract, the bidders should obtain original power of attorney from the area agent for project area and copy power of attorney of manufacturer to Chinese area general agent or project area agent and/or copy power of attorney of Chinese area general agent to project area agent with the bidder’s common seal in the same time. 4, one piece of copy of the purchases person's ID card with the bidder’s common seal.
